Hazard Door Signs

Laboratory Animal Resources Animal or Procedure Room Door Signs

There are four signs and one door card developed by Laboratory Animal Resouces (LAR) to simplify the door signage required for notification for the presence of hazardous materials administered to animals. If you are working with a biological, chemical carcinogen, Drug/Toxin, or radioactive isotope hazard within LAR then these are the signs that must be posted. For animals infected with similar hazards in a room only one hazard door sign is needed however, a contact/precautions card is needed for each investigator so the contact information and safety precautions can be indicated for each investigator. This eliminates the need for multiple door signs of the same type. These must only be posted while the hazard is present, once the hazard is removed the door sign can be turned around or taken down and reused later. Make sure your sign is updated periodically by changing the original posting date. This way LAR personnel know that the sign is current. UW Safety Department Hazard Door Signs For Laboratory Use

The following signs were developed by the UW-Madison Safety Department for notifying personnel of the presence of hazardous material. Those files ending with "-F" are fillable on-line before printing. The others are also fillable by using the typewriter feature on latest version of Adobe Acrobat 9.0 Profesional.

Labels for Animal Cage Cards

The four main default labels 1) Biological Hazard, 2) Chemical Hazard, 3) Drug/Toxin, and 4) Radioactive Isotope Hazard are for attaching to an animal cage's "cage card" so personnel are aware that there is a hazard present. The other labels below can be used when they more appropriately indicate the hazard. All the labels listed below are printed out on an 8.5" x 11" sheet with a total of 80 labels per page; each label is 1/2" wide x 1-3/4" long. They are available through MDS - Corporate Express Office Supplies catalog no. AVE5267. Any label can be used as long as the appropriate hazard symbol is used with the name of the agent used. A number of these are also available in the animal facility. If you don't see them ask the area superivosr or lead or call JoAnne @ 2-0456.

Labels and MSDSs for Bottles of Chemicals Used in Secondary Containers

The labels listed below are for the most frequently used chemicals in LAR that are used by all those who handle animals. The materal safety data sheets (MSDS) are provided. Call 2-0456 if there is a label or MSDS that you need.

LAR Safety Equipment Certification Dates

Laboratory Animal Resources has all of their Biological Safety Cabinets (BSC) and Clean Air Devices (CADs) recertified by Environmental Health and Safety annually. Researchers completing a safety protocol for the Office of Biological Safety (OBS) need to identify the safety cabinets they will be using and their current certification dates (III. Location) in the Vivarium. Clicking on the title of this section will take you to a pdf file with the certification dates of all LAR cabinets in the Vivariums. This form will be updated as new information or certifications are received. Remember although the CADs are listed on this form they are not to be used for biologicals, chemical hazards, hazardous drugs, or radioactive isotopes; CADs are only intended to protect the animals from you and vice versa.

Note: Virkon-S is corrosive on stainless steel so should be wiped off as much as possible after use. If you don't remove the Virkon-S the stainless steel will deteriorate faster than "normal" and the unit will not last as long as it should.
